Session description: 

The majority of people at risk of malnutrition live in the community and early identification and treatment or prevention is the best approach. However, community dietetic services rely on referral by other health professionals, who may not always have the knowledge and skills necessary to identify those in need for nutritional support at an early stage. The session will outline a service model for supporting frail and malnourished people living in the community, by dietitians working within a general practice (family doctor) multi-disciplinary team. The dietitian in this case study was able to use data held in general practice systems to screen and identify those potentially at risk of malnutrition and then intervene to prevent deterioration. A brief update of current and future initiatives being undertaken by CMTFs Primary and Community Care Working Group will also be shared.

About the Speakers: 

Mary Hickson PhD RD has been Professor of Dietetics at the University of Plymouth since 2016. Her research interests include all aspects of dietetic practice, sarcopenia and frailty, hospital nutritional care, and nutrition in older people. She worked in the NHS in various roles for over 23 years bringing extensive clinical experience to her academic role. She is also passionate about supporting dietitians to do high quality impactful research which enhances the profession’s evidence base. She has supervised six dietitians to obtain their doctorate and she has three more on that pathway. She is actively mentoring and supporting several others to gain funding to do a PhD. The work Mary will be presenting starts to form the evidence for dietetic practice in primary care in the UK. She is seeking funding to continue to develop this programme of research to ensure dietitians can work effectively and efficiently in this setting. 

Avril Collinson PhD RD works as an Associate Professor and Academic Lead for Dietetics at the University of Plymouth. She joined the University in 2008 having worked in many different specialities in the NHS for over 14 years; specialising in gastroenterology and renal.  Her research interests focus on dietetic practice both in primary and secondary care. She has recently been working on evidence for dietetic practice in primary care in the UK. 

 

 

Heather Keller RD PhD is the Schlegel Research Chair in Nutrition & Aging at the Schlegel-UW Research Institute for Aging and University of Waterloo. She is an internationally recognized expert in geriatric nutrition, assessment, and treatment. Research areas are focused on nutrition risk and malnutrition identification and treatment across health care sectors; improving nutrition care in hospital; self-management and using screening in the community to improve nutritional status; institutional food and promoting and improving food intake, especially for vulnerable older adults, including those living with dementia. Professor Keller has led several national research and knowledge translation projects and published over 250 peer-reviewed articles and translates much of this evidence into practice with tools and resources. She is past chair/co-chair (2009-2018) of the Canadian Malnutrition Task Force and currently leads the Primary Care working Group of CMTF.
